Camp Health Paperwork
As you work to complete your child’s health forms, know that it is time well spent! The Aloha Foundation health staff relies on the information you input to provide safe and appropriate care for your child. Thank you for taking the time to read through the details below and to ensure your camper’s paperwork contains accurate and thorough information.

Required Documentation
Health History Form: Available in your online account on January 1 and due April 1, this form must be completed by a parent or guardian on an annual basis. A physical exam or doctor’s report is not required.
Vaccination Policy: The Aloha Foundation requires all participants at Aloha, Hive, and Lanakila to adhere to our vaccination policies and upload proof of vaccination to their records.
Health Insurance: Participants are required to be covered by a comprehensive health insurance plan and must upload photos of the front and back of their insurance cards, including dental, vision, and separate prescription cards if applicable. Families are responsible for all medical costs not covered by their insurance. This includes co-pays, deductibles, and prescription charges. International families: All medical bills will be forwarded to you. Please pay the provider directly and then submit bills to your insurance company.
Medication Order Form: If your camper will take medications at camp, this form must be completed and signed by the prescribing doctor. Learn more about Medications at Camp.
